Undersigned agrees that the above-named dog, herein referred to as dog(s) will be trained to perform as agreed in "Explanation of Services" section of this document. Undersigned acknowledges that training exposes their dog(s) and themselves to certain related risks such as but not limited to being bitten by a dog, scratched, knocked down, jumped on, frightened or otherwise injured. By signing below I assume all risks. I hereby release Danielle Doth and Elkhorn Dog Services LLC from all responsibility in case of injury, death, loss or damage to myself or my dog. Danielle Doth and Elkhorn Dog Services LLC. will not be responsible for any negligence on my part and I will accept responsibility for my own actions as stated by Kentucky laws. At its sole discretion, Danielle Doth and Elkhorn Dog Services LLC. (or any authorized agent thereof) may provide special training whereby my dog(s) is transported by automobile from the housing premises located at 224 West Stephens Street Midway Kentucky 40347. During said special training, my dog may be allowed off leash. I hereby release Danielle Doth and Elkhorn Dog Services LLC (and any authorized agent there of) from all responsibility in case of automobile injury, death, loss or damage to myself or my dog. I understand the risks related to off leash training, particularly including, but not limited to escapes, and notwithstanding consent to this special training. In the event my dog escapes, and Danielle Doth and Elkhorn Dog Services LLC is unable to locate or retrieve my dog, I agree to hold harmless Danielle Doth and Elkhorn Dog Services LLC from claims, suits or any other such action resulting therefrom. If the dog shows evidence of a medical problem while under the care and control of Danielle Doth and Elkhorn Dog Services LLC they shall have the right to take such measures as they deem necessary, without being liable to owner for the health and safety of the dog, including the right to administer medication or obtain an evaluation and treatment by a veterinarian of Danielle Doth and Elkhorn Dog Services LLC's choice. If, in the opinion of Danielle Doth and Elkhorn Dog Services LLC or the veterinarian selected by Danielle Doth and Elkhorn Dog Services LLC the cost of treatment may exceed the amount $50.00, reasonable attempts will be made to contact the owner. In the event that the owner cannot be contacted, Danielle Doth and Elkhorn Dog Services LLC. or the veterinarian selected by Danielle Doth and Elkhorn Dog Services LLC may provide such emergency medical treatment as is deemed necessary to preserve the life and health of the dog. Owner shall pay the cost of all treatment provided, in addition to the contract price, prior to the dog being picked up by the owner. If, during the training of the dog, the training is interrupted for any reason not the fault of Danielle Doth and Elkhorn Dog Services LLC, including but not limited to the illness or reproductive cycle of the dog, the owner will be notified by Danielle Doth and Elkhorn Dog Services LLC, and the owner agrees to make arrangements to have dog removed from Danielle Doth and Elkhorn Dog Services LLC as soon as possible. If any delay and interruption in training is the fault of Danielle Doth and Elkhorn Dog Services LLC, including delays for stress which may show up during training, owner shall not be charged for any such delay or interruption. Danielle Doth and Elkhorn Dog Services LLC agrees that while the dog is on its premises and under its control, Danielle Doth and Elkhorn Dog Services LLC will exercise reasonable care to protect the health and safety of the dog. Dogs in a board and train program shall be fed properly and regularly, shall be housed in their own quarters, shall be brushed and cleaned as needed. However, owner agrees that Danielle Doth and Elkhorn Dog Services LLC has no liability for loss or damage to the dog or owner’s other property resulting from fire, disease, illness, virus, escape, theft, death or other causes. After the dog has been returned to the owner by Danielle Doth and Elkhorn Dog Services LLC, Danielle Doth and Elkhorn Dog Services LLC shall not be responsible for the illness, injury or death of the dog or of any person or animal, or damage to any property, caused by the dog. Aggressive behaviors include but are not limited to; growling, chasing, snapping, or biting that resulted in scratch marks, bruising or punctures. Undersigned also acknowledges that there is no guarantee in regard to resolving aggressive behavior, and will, if recommended by Danielle Doth and Elkhorn Dog Services LLC, agree to muzzle dog when in public for the safety of other dogs or people.
